A Photo ID

Copy of last year's tax return (if you have it)

Social Security numbers or Individual Taxpayer Identification Numbers (ITIN) for you, your spouse, and any children born before December 31, 2007. You must bring the actual cards or a copy of the cards.

Correct birth dates for everyone listed on your return

Income documentation, including:

W-2 forms from all jobs worked in the past year

All 1099 forms showing other income received last year

A blank check for direct deposit of your tax refund (If you don't have a checking account, bring your bank account name, number, and the 9-digit American Banker's Association (ABA) routing number. Contact your bank if you need help identifying the routing number)

If any of the following apply to you, take documentation with you:

Child care expenses, including the provider's name, address, and tax ID number (Employer Identification Number or Social Security Number)

Mortgage company statements

Adoption expenses

Alimony paid or received

Any notices received from the IRS

Property tax bills

Amount of unemployment insurance you received

Amount of interest you earned on a bank account

1098 forms for student loan interest and/or college tuition

1098 forms for interest paid on a home mortgage

Additional forms of income, such as:

Prizes and awards

Scholarships and fellowships

Winnings from lottery/gambling

If you lost or do not have all of these items, you can still have your tax return prepared. Call the IRS help line at 1-800-829-1040 to find out what you need to do and how to order replacement documents.
